You can schedule your own appointment through the self scheduling feature in our EMR system.
Please read the information below BEFORE you click the link below to go the scheduler.
Chris Dukarski,PT, the owner of OrthoWell and WalkWell, will fabricate a pair of custom foot orthotics for you at designated times that you can choose from in both Beverly and NBPT.
In Beverly, we have appointments on Wednesday ONLY at 9:30, 10:30, 11:30, 12:30, 3:30 and 6:30.
In NBPT, we have appointments on Tuesday and Friday at 3:00.
Please click on the designated days above and the system will give you a list of available times. It no times populate, then we are booked for the day. Please select another day.
Don’t forget to bring a pair of work shoes, a pair of sneakers, or a pair of dress shoes depending on what shoes you need orthotics fitted to.
We accept all forms of payment: personal check, cash, credit cards or flex benefit card.
PLEASE bring a copy of our directions as the Cummings Center is a big place! and you WILL get lost without the directions.

When you click on the CLICK HERE button below, please choose the Register + Schedule button in the “I am a new patient” section if this is your first time. If you are an existing patient ie an active patient or have been to see us for treatment in the past several years, then choose “I am an existing patient” and click Schedule an Appointment.
MAKE SURE that you choose ORTHOTICS as your insurance and enter N/A when asked for insurance number. This will bypass the insurance info that we need for physical therapy. Your insurance company does NOT cover custom foot orthotics.
NOTE: Please give us the courtesy of a 48 hour notice before any cancellation so that we can fill your time with another patient who is on our waiting list. If you cancel without 24 notice or do not show for your 1 hour scheduled appointment, then you will be charged a $50 cancellation fee. This will be added to the cost of your orthotics if you decide to reschedule. The only way to cancel within the 24-48 hour window is by calling 978-522-4199.
